Аннотация:

Работа посвящена возможностям мультиспиральной компьютерной томографии (МСКТ) в диагностике и стадировании острого пиелонефрита (ОП) для изучения роли почечных аномалий в развитии ОП и выбора тактики лечения. У 59 пациентов с ОП при подозрении на гнойно-деструктивные осложнения была выполнена МСКТ, у 7 больных она была произведена дважды для контроля эффективности лечения. Исследование установило, что ультразвуковое исследование, а также экскреторная урография не всегда информативны в выявлении гнойных форм воспалительного процесса, возникшего на фоне пороков развития почек, особенно аномалий формы, расположения и структуры. В результате сформулированы показания к выполнению МСКТ у больных ОП. Улучшение диагностики, достигнутое с помощью МСКТ, позволило уменьшить количество операций и избежать нефрэктомий.

Аннотация:

Радиочастотная абляция является малоинвазивным методом локальной деструкции, широко применяемым в комбинированном лечении рака почки. Применение радиочастотной абляции позволило расширить показания к более радикальному лечению новообразований почки у пациентов, которым не возможно выполнение традиционной нефрэктомии или резекции почки, вследствие крайне отягощенного соматического статуса

Эффективность и безопасность РЧА значительно повышаются при ее сочетании с предварительной суперселективной окклюзией сосудов, питающих опухоль. Мы обладаем опытом применения суперселективной эмболизации в комбинации с РЧА при опухоли почки у двух пациентов. В обоих наблюдениях получен хороший результат комбинированного лечения.

У ряда пациентов такая комбинация (ССЭ+РЧА) может являться альтернативой открытой операции, расширяя тем самым арсенал современных малоинвазивных методов лечения опухоли почки.
